How to Refer a Patient
Download and complete the appropriate referral form below.
Submit the completed referral form by secure fax at (937) 912-5159 or by secure email as indicated on the referral form. Please include any relevant office notes, medication history, and supporting clinical documentation when available.
Our clinical team will review the referral, contact the patient to coordinate scheduling, and communicate with your office as appropriate throughout the referral process.
